GP Improved Access Appointments
What is the GP Improved Access Service?
The five Farnham practices (Downing Street, Farnham Dene, The Ferns, Holly Tree and River Wey) continue to work together and will be offering routine appointments to their registered patients through NHS England’s new Patient Improved Access.
The Improved Access appointments will give patients from all five practices the opportunity to see a doctor, nurse or health care assistant for a routine appointment outside the core hours of their GP practice.
The appointment with the doctor, nurse or healthcare assistance is booked through the patient’s home practice reception team. However, the appointment will take place at the Farnham Integrated Care Centre based in Farnham Centre for Health.
Patients are also able to book a routine GP appointment on a Sunday morning through the patient’s home practice. These appointments will take place at Frimley Park Hospital, Out-Patient 1.
When are these appointments available?
Appointments are available from 6:30pm and 8:00pm , Monday to Friday and 9:00 am to 12;00 noon on Saturdays - excluding bank holidays.
Appointments must be booked in advance.
Who can I access these appointments?
Anyone registered as a regular patient with Holly Tree Surgery.
How do I book an appointment?
Contact your GP practice in the usual way to request an Improved Access Appointment.
If urgent care is required please call ‘111’ for further advice. If your need is life threatening, please go to A&E or call 999.

Will I see my own GP and/or nurse?
You will be seen by one of our GPs , but it is unlikely that you will be seen by our own nurse or healthcare assistant. If you would prefer to see your usual GP please attend the practice during normal opening hours.
Will the GP I see have access to my medical record during my appointment?
Your medical record will be available to the GP that sees you, to ensure that they have the information they need to give you the best possible care. Notes of your appointment will be recorded directly into your medical record to ensure that your record is up-to-date. An appointment with this service will be just like an appointment at your own GP practice.
How do I cancel an appointment?
If you need to cancel the appointment, please call our reception team stating your name and the time and date of your appointment with the GP Improved Access Service.
Where do I go for my appointment?
The service is located within Farnham Centre for Health. When entering the building from the main car park, once through the entrance, turn right and you will see the door into Farnham Intergrated Care Services (FICS) on your right.
