Telephone Advice Lines for Patients Recently Discharged from Hospital
Frimley Park Hospital NHS Trust has introduced three dedicated telephone lines for patients to call if they have any queries or concerns within 28 days after they leave hospital. Patients are given the relevant telephone number along with their discharge information. This initiative forms part of the work the trust is undertaking to help reduce readmissions and improve patient experience.
