Surgery afternoon closures..

Please note that on the following dates out medical staff will be attending important training events arranged by the Clinical Commissioning Group (CCG).  The surgery will be open as normal in the mornings and the Reception and Dispensary will be open during the afternoons until 18:30.  There will be no medical staff on site on these afternoons.  If you require medical assistance in the afternoon on one of these dates please telephone 111.

The NHS Friends and Family Test (FTT)

The Friends and Family Test is a short survey asking patients whether they would recommend the NHS service they have received to friends and family who need similar treatment or care. This valuable feedback will be used to improve services and help patients make informed choice about the NHS provider they use. Results will be submitted to NHS England on a monthly basis and will be published at a national and local level.

Holly Tree Surgery would be very grateful if you could take the time to click on this link and fill in our Friends and Family test at  Alternatively a paper copy can be filled in at the surgery.

Named Accountable GP

The Government requires all patients to have a named accountable GP.


Every patient at this surgery has always had a named accountable GP, and this will be one of our three partners (Drs Adams, Elliott or Price).


Having a named, accountable GP does not stop you seeing any of the doctors working at the practice.


Having a named, accountable GP helps hospitals and other health care providers send information through to us quickly and efficiently.


If you are already registered with us, our reception staff will be happy to confirm who your named accountable GP is if you do not already know. If you are registering or re-registering with us we will inform you who your named, accountable GP is at the point of registration.


If you have a preference as to your allocated named accountable GP, then please speak to us and we will try our best to assist.


Please remember this doesn’t stop you from seeing whichever GP is on duty or you may choose to see. 

Online Appointments with our Health Care Assistant

The following appointment types can now be booked online:

  • Routine blood tests which have been requested by your doctor
  • Blood Pressure Checks
  • Ear wash outs.  Please note you will need to have been administering olive oil drops into your ears for at least a week before your appointment date.  Please indicate whether it is one or both ears that need syringing.

Please click the link below to register for online access.  You will need to bring ID with you when you attend your appointment to complete the registration.  Alternatively bring ID to the surgery and ask for a registration letter.  This will also allow you to order repeat prescriptions online.

If you are already registered to order repeat prescriptions online then you will automatically have access to the online appointment system.

Patient Consent Forms

We would like to obtain consent from our patients to leave messages on landlines/mobiles or via email. Please sign a consent form when you next come to surgery. They are available at reception.
